Feedback on Continuity of Carer Maternity Services in Great Yarmouth and Waveney
We are asking pregnant people, new parents, co-parents and birth partners to tell us about their experiences of our new Continuity of Carer model of care. Our first wave of Continuity of Carer teams were launched on 1st February 2021, with plans to fully implement the model of care at JPUH by the end of the year.

This information is being collected by Birth Voices East to gain a better understanding of current local maternity services so that improvements can be made in the future. We will process your personal information in order to deliver this service safely and effectively, and where otherwise reasonably necessary for our purposes.
You can expect your personal information to be stored securely. It will be kept on our systems for as long as is necessary for the relevant activity. We will not share your personal information (contact information) with anyone else without your explicit prior consent. Your responses will be anonymised and only used for the purpose of improving local maternity services.
